This is a daily behavior ticket that I use in my primary classroom. Every student puts their name and the date on a ticket first thing in the morning and keeps it at their desk. Throughout the day, if misbehavior occurs, and warnings are ignored, a smiley face is crossed off. The student must return the ticket the next morning signed by their parents. Students bring back their signed ticket even if they have not had any smilies crossed off. Daily positive feedback to the parents that their child had a good day is as important as letting them know when they don't. I print this off on super bright (neon) paper so it is not easily misplaced.
