In my classroom, I print weekly Exit Ticket pages front and back. I close each lesson with these Exit Tickets, beginning with students rewriting the learning objective in their own words (I require use of new vocabulary). Students then rate their understanding on a scale of 1-5, which is helpful when students are more shy about asking questions. They then complete two problems related to that day's lesson. Sometimes I will make it one tough question and give them both boxes.
There is also a "Bonus" section dedicated to early finishers (Perfect time to incorporate some rigorous writing!).
