This product has four forms you can use in the classroom. One form is to organize bell work only. A second form is a log sheet for recording the daily objective, self-assessing, and completing the exit question/problem. The third form has a section for the objective, the bell work, the exit problem/question, a scrap area for miscellaneous problems or comments. The fourth form is designed to be cut in half to save paper. It is for 5 days of bell work (2 on the front, and 3 on the back side).
Currently, I use the Bell Work Exit Ticket Form everyday so that the students are aware of the objective for the day, have a place to complete the bell work. self assessment, and exit problem, and have a place for work that is done in the classroom. This sheet is copied back-to-back to save paper. Any part of the content may optionally be written in the students' notes at the discretion of the teacher and student. Sometime during the class period I will ask the students how they feel they are coming along with respect to the daily goal, and they can record their self-assessment on the log. The students may also write what they need to improve their progress. I collect the Bell Work Exit Ticket Form so I can assess the students' learning for the day.
