This form can be used to list students in a class and keep track of what happens in class each period of the day. My instructional assistants and I use this to mark homework completion, note the day's homework, and list any other information that may be important from that period's class.
I list the students' names, teacher, period, subject, and date at the top. We also staple copies of the class' assignment to this sheet. I keep a binder each trimester for all homework sheets used.
This is helpful not only for classes that I don't attend, but for behavior notes on students.
