A daily reading summary homework form on the front. Students record the book, genre, level and write how many pages they read for each day. They write a who did what when and where summary and then parents can initial.
On the back is a skills based form where students must do each of the following for their independent book each week:
Make a connection
Identify author's purpose of a chapter and explain how they know
Identify one of 5 common text structures and explain how they know
Sequence 3 events
Draw a conclusion with two supporting facts
Draw a visual that advances the plot.
It works for any genre! I do change the visualize the plot section to having students identify fact and opinion when they choose nonfiction.
