Students color in the square with their behavior color they end the day on. Then at home students read to someone for the time limit you allot and then the parent/guardian sign off on it.
I give this out every Friday and it is due every Friday. If a student is absent I just have them cross out the box.
I use a 5 color scheme behavior plan where student start in the middle and move up for good behavior to receive rewards and they can move down for less desirable behavior and they receive a consequence.
You could also use this for a specific student if you wanted to do a positive behavior plan or reading intervention.
