I've used this and it works great. Daily, you are able to keep track of subjects, objects in progress, if completed/incorrect/refused/# of (total)trials, and a space to document how the task was completed (independently, VP, Modeling, GP, H/H). If you have different teacher assistants working with your students, there is a space available for their initials so you can know who worked with what students that day.
