I have created and used these in my classroom to help support positive behavior in students. Typically I have conversations with the students and we fill this out together. This conversation helps the student be aware of the choices they are making.
Each behavior chart is a little different and can be used for a variety of purposes: executive function, time on task, work completion, recess behavior, classroom behavior, etc.
