I have used this checklist for students who need some extra behavioural monitoring and support. Each day I use this checklist to track target behaviours and communicate with parents about how the day went. You can copy the page double-sided and cut it down the middle to create a "notepad". I found that this checklist, accompanied with regular conferencing, helped the child to become more aware of their behaviours. It is framed in a positive, strength-based way (i.e. what they doing, rather than what they are not doing). I have included a blank checklist so you can write in goals specific to the child.
