Data Binder and additional Progress Monitoring Resources
This packet contains items that may be used to monitor and
track student progress on a daily, weekly, or monthly basis. The majority of forms are editable and when applicable include a blank template for that specific form.

Data Binder and Progress Monitoring Resource includes the following:
- Binder Covers:
Three border variations that apply to grade levels kindergarten
through fifth grade.
Each border includes a reserved space to input student's name.
- Assessment Data Tracker and Progress Monitoring
Includes an available space to input student modifications and
accommodations, particularly those relevant to testing.
Monthly Assessment Chart. Input student test scores each month
for the following assessments: STAR Math, STAR Reading,
STAR Literacy, standardized ELA assessment, standardized math
assessment, current reading level, sight word recognition, and
other. Assessment chart was created by me and is in no way
affiliated with the Renaissance/STAR program. Blank template
provided to fit your needs.
- Assessment Data Tracker and Promotion/Retention Record
Record grade level promotion requirements and retention
policies. Can be used in conjunction with the previous form and
data. Document whether student is being promoted, placed, or
retained to a specific grade level. Blank Template Provided.
- Goal Setting Record
Drop down menu allows you to make form specific to subject
an/or content area.
Student and teacher will collaborate and identify student's specific
strengths and weaknesses.
Student and teacher will work together to create and record a
specific and obtainable goal set for a specific period of time.
After that time has passed student and teacher will identify
whether the student has obtained their goal and explain.
- Self Assess Graph (Sight Words)
Student and teacher will identify number of words the student
recognizes and the student will be responsible for recording the
amount of words known by coloring/charting their progress on the
graph. Graph is applicable to both the Fry and Dolch sight word
list. Graphs included are for monthly and bi-weekly progress
monitoring.
- Editable Sight word fluency progress monitoring line graph.
Input your student data and watch the graph plot your student's
progress. Graph is applicable to the Fry, Dolch, and Snap word
lists. Chart your student's progress throughout the year. Note
whether your students are accurately tracking while they read.
- Individualized Education Program (I.E.P) classroom data tracker.
Record each of your students that currently have an IEP. Input
their I.E.P renewal date. Use the drop down menu to select their
qualifying disability. Identify (check) whether they have or need the
following: Instructional Accommodations, Classroom testing
accommodations, supplemental aids and services, speech
services, occupational therapy, resourcing, attendance in a pre-
kindergarten program, previous retention, behavior intervention
plan, psychological evaluation, GAA, medical concerns, and other.
- Student Service Providers Service Log
Record team services provided to your students and the frequency
Monthly calendar allows you to identify dates your student is
receiving services, includes: Occupational Therapy, Speech,
Resource, SPED, Psychologist, Nurse, Other.
- Student Service Providers and Team Member Tracker
Input team members, days and times to be serviced. Includes:
classroom teacher, special education teacher, resource teacher,
school psychologist, speech therapist, occupational therapist,
physical therapist, school nurse, school administrator, child
advocate, and other.
Blank template is provided to meet your student’s individual needs
- Behavior Monitoring
Track your student’s behavior. Includes: time behavior occurs,
activity during which the behavior occurs, antecedent, and length
of time the behavior lasted.
- Behavior Monitoring Chart
Rule or objective you wish to observe, whether it was followed or
not followed, and the time and antecedent.
- Behavior Monitoring Chart (Teacher led or self-assessment)
Record individual goals, whether that goal was met or not met,
as well as, the time and antecedent.
- Behavior Monitoring Chart (Self-assessment)
Use drop down menu to select subject or content area.
Input time of day for the selected subject.
Discuss with student during each subject how they did and have
student highlight or circle which face best applies to the behavior
they exhibited.
- Letter Recognition Assessment
Assess the lower case and capital letters student recognizes.
Circle or highlight letters students recognizes
Record on Letter Recognition Progress monitoring form
- Progress Monitoring Letter Recognition Data
Record all the lower case and capital letters the student recognizes
Once a student identifies a letter, assess them on that letter for a
minimum of three attempts. Once they have identified that letter
on three separate occasions, record date of mastery.
-Sound Recognition Assessment
Assess students on the sound each letter makes, including long
and short vowels.
- Progress Monitoring Sound Recognition Data
Record all the sounds the student correctly knows for each letter.
Once a student produces or identifies the sound for each letter a
minimum of three occurrences, record mastery date.
- Number Recognition Assessment 0-100
Circle or highlight the numbers student is able to name or identify
Use one of three number grids to assess student’s ability to count
to count to 100. Each grid has a different set of numbers that has
been omitted that the student is required to fill in.
There are 3 variations to this assessment with a different set of
numbers omitted.
- Number Recognition Assessment 0-120
Circle or highlight the numbers student is able to name or identify.
Use one of three number grids to assess student’s ability to count
to 120. Each grid has a different set of numbers that has been
omitted that the student is required to fill in.

numbers omitted.
- Editable Sight Word Progress Monitoring Bar Graph Monthly
Input student data into editable template and watch graph record
the information in the form of a bar graph.
There are two separate graphs, one for the Fry word list and the
other for the Dolch word list.
There are two additional graphs for bi-weekly progress monitoring.
- Weekly Assessment Data (Self-Assess)
Drop down box allows you to pick the subject or content area.
Each week the student and teacher will discuss grades received
on their weekly assessments. Student will then be responsible for
coloring their graph to show their current score or grade. Student
will be able to see whether they are improving or still weak in that
content area.
The school year is divided into two segments; before and after
Christmas. (Weeks 1-18 and 19-36)
Blank template provided to address the needs of your students.
- STAR Reading and STAR Math Progress Monitoring Bar Graphs
Input your students scaled scores into an editable template and
document their performance and growth.
This graph is in no way affiliated with the Renaissance or STAR
programs. It is self-created to record that data in the classroom.
- Accelerated Reader Data Chart (Self-assess)
Student will be responsible for recording the title of the book they
read, as well as, scores received. They are also responsible for
checking off that they have read the story for a total of at least
three times and are ready to test.
- Tier II or Tier III Response to Intervention Documentation
Record student data, interventions, and strategies used as
interventions to help facilitate student learning and document
student progress.
- Classroom Unit Test Data Record Chart
Input the names of each student
Using the drop down menu select the subject area and unit test
number for which you wish to record data.
Enter student’s pre and post test scores
Identify if student has mastered the assessment or unit
Document growth
- Lexile Reading Level Chart
Identifies correlation between color, reading level, grade level, and
text complexity.
