This document can be used to help students self reflect on any type of behavior goal. Good to use for conferring with student and to use as a tracking device with behaviors. Also good for parent communication and student records.
There is an area to write the goal down and choose an area that their goal is focusing on (example: hallway). There is an area to choose a accomplishment date and then a space to record their reflection by shading on a 4-3-2-1 scale.
