If you have skills trainers, para professionals or other school staff that collect data, this is the last data sheet you will ever need.
The first page is a data sheet template to input any IEP objectives. Data can be collected by percentages or by frequency. The key at the top also gives the option to collect data based on prompting hierarchies. The cells are no locked so that you can make them bigger or smaller, depending on the need of the objective. Up to 50 objectives can be tracked by this document.
The second page is the analysis page. IEP objectives that were inputted into the data sheet are automatically transferred to this page. Data is inputted and is automatically graphed so that it can be analyzed and shared with other educators or parents. There is also an area to input comments. Each objective has up to 5 additional data points that can be graphed as well. For example if graphing behavioral data, different intervention phases can be plotted in the same graph. Also, if graphing academic objective, related objectives can be graphed on the same graph (wh- questions, different multiplication fact families, etc...).
This data sheet is very flexible and data can be collected very easily and quickly. If there are unused objectives in the excel document, those cells can be hidden.
This form has been tweaked and adjusted but I have been using this template for over 5 years and have not needed another one.
