Some therapists use address labels to document therapy services- especially when seeing several students in a group. This helps eliminate flipping pages and going back and forth from one data sheet to the next. This is a data sheet that can be kept in a notebook or folder and the sticker labels can be put on these sheets after the therapy session is over. There is a box at the top to include therapy goals as well as spots below for the date and for the labels to go. These fit the standard (Avery brand or other brand) Address Labels (the ones that are 1" by 2 5/8" in size). Avery brand 8460, 5160, or 3M brand 3200-B or 3100B.
