This data collection form is editable. It gives information about how independent the student is traveling throughout the classroom to his visual schedule. It clarifies how much and what type of support is needed from an adult during transitions.
The second page is a scatter plot to check in on student's behaviors that are listed in the child's behavior intervention plan to check and see if the planned interventions are working to successfully decrease the behaviors. It asks: What behaviors are still happening? How often are they happening? Are there any new behaviors that have been observed?
