I usually have a conduct sheet for each child that lasts for a month, but I've had a few children who have needed a more detailed conduct sheet. This helps inform their parents the exact behaviors their child is displaying in the classroom. I kept this sheet with me at all times and put a check or an x in the box next to the behavior whenever I saw it occuring as long as the time it occurred. This is great for documenting. I would NOT advise doing this with every child, only the extreme cases.
