I created this detention notice for use on our campus. We print the English form on one side and the Spanish form on the back. The form is easy and quick to fill out, and allows for teacher and parent comments, as well as a place for the parent to request further discussion about the incident.
I make the students copy the apology letters in detention. When they are finished copying the letter, they read what they wrote aloud to me while I look at the template and follow along to make sure they copied everything. I keep the letters on file. If the student is apologizing to another student, I make them give the letter to that student. If the letter is for a sub, I make the student take a copy home to be signed by their parents and make every effort to give a copy of the letter to the sub.
