This classroom management tool changed my teaching forever! I have the document run off on two-part NCR (No carbon required) paper. These are then cut into the four slips. I always have some on my clip board when in my classroom or around campus.
Prior to this, I struggled with the paperwork and logistics of assigning and monitoring after class detentions. I created this document as a means to help with all of the above and more.
When a student has done something that warrants a detention, I quietly fill out their name, the date, the reason, and sign it. As soon as the time presents itself, I speak individually with the student and tell them they have been assigned a __ minute detention for whichever reason. Then I ask them if they can come in today or tomorrow, etc. I circle the day, write down their response (before school, at break, etc.), and then give them the original. I keep the copy.
The beauty of this system, is that when the student serves the detention, I write across my copy that it has been served and the date. Now I have a record of detentions served with me. If a student doesn't serve it, I remind them the next day and give them another opportunity. But now I have a paper trail for the next step of discipline process, when needed.
