I designed these tables to use as a record of my teaching and observations during my small group lessons during rotations. In ELA and Math, my fourth grade class is separated into five different groups based on ability. As a way to plan for my differentiated small group teaching, I write brief notes on what my teaching point is going to be for the day.
I print these sheets double sided and have them ready to go in my small group planning binder. In the first column, I record the date and the students in the group to remind myself whom I was meeting with that day.
During my meeting or directly afterward, I take notes on how they do. This way, I can easily glance back at my notes to know what I need to focus on during our next lesson. It's amazing how much more I can keep track of using these notes! I can also write down specific observations about individuals so I remember to touch base with those kids later.
