Show you care about each child and value guardians' input through this editable, open-ended parent survey that is made up of 12 thought-provoking questions. Two versions of the survey are included in this free resource: English and Spanish.
I've used this parent survey with my students for several years, and the feedback it provides me has been invaluable. When I meet with parents at conference time, we often refer back to answers they provided on these parent-surveys. Surveying students is important, but perhaps even more important is getting feedback from the people who know the tweens and teens the best -- their guardians.
