This product is an online information form for your students and their parents. This Google Form is completely editable, so if you don't like something or want to add something, you have to freedom to do so!
The beginning of the year can be stressful for parents, especially with all of the paperwork that they have to fill out by hand! Make their lives easier by giving out your info sheets online!
There are 5 sections in this form:
-Contact Info
-Volunteer Opportunities
-About Your Child
-Transportation
-Final Thoughts
Please note that this is a Google App, which means you must allow TPT access to your drive in order to get access to this product.
