I have uploaded documentation forms for use in taking anecdotal notes on student performance under measures of the DRDP-K that do not lend themselves to checklists. I have also included common assessment checklists for those measures that can be documented using checklists.
As a way of organizing my documentation, I made a folder for each domain under which I will be observing my students (in Louisiana we are observing only these domains: ATL-REG; SED; LLD; ELD; COG-MATH; and PD. (I have included.only these domains in my forms)
I first typed my name in the header on each documentation form. I then printed the documentation forms and made four copies of each so I would have enough pages on which to document anecdotal notes on my 25 students. I wrote each student's name on the lines I included next to the measure names. (This took me a little while, but I did not want to try to type names on the small amount of space I had for each line after I typed in the measure description). I was afraid it would compromise my document.) I will be using three different-colored sticky notes on these forms (one color for my first observation, another color for my second observation, and, finally, a third color for my third observation of each measure). Of course, I placed each form in the domain folder with which it coordinates.
For the checklists, I made a class list (students' first names only) on a separate sheet of paper setting my font at 12. I copied and pasted my class list into the common assessment forms so I would not have to write names by hand.
