This is a really simple, really straight-forward self-assessment that I give students after their first rehearsal work period in drama.
It outlines what I'm looking for every time I mark them for rehearsals - no matter WHAT their project/unit.
It's an eye-opener...how honest they are with themselves, and how this helps them "check in" and see what they need to work on.
There's a mark, that you could either count, or not, depending on what you need.

NOTE - there's two rubrics on one page so you can cut it into smaller slips so there's less photocopying. I often have a bunch of these handy in my class because I do these check-in's more than once.
Sometimes on the back of the slip I make them take one of their areas that has the lowest mark and write a comment on the back about how they're going to work on it.
