I originally made this for a couple of students I was tutoring who were having difficulties dissecting word problems dealing with fractions. The first page acts as a quick review on simple fractions -- students have to divide the shape up into a different number of equal parts, then provide the fraction as well as the fraction in words.
The second page requires students to draw an actual diagram based on the information that is given. I found that this has helped them really pay attention to what the words are telling them -- as opposed to deciding to just add/subtract fractions together for an incorrect answer.
Let me know what you think of these if you end up using them for your classroom!
