● Binder covers:

● Daily Data Log
: Includes space for goals and IEP info for 1 student. Color border and B&W border versions.
● Medical Assistance- compatible data log:
Includes signature line and NPI/license number line for MA compatibility. 5 rows per page, so it’s easy to see when your progress report is due (every 10 visits). Color border and B&W border versions.
● Services Info page:
Includes blanks for 2 providers with contact info, schedules, and goals; a list of provider types to circle; and a place to write Strategies/Cues that work. This is great for leaving with teachers and daycare workers! Color border and B&W border versions.
● IEP Due Dates by Month

● Parent notes
: I send home these half-sheet notes after each session. Includes check boxes with common goals/activities as well as lines to write other notes. One page each (2 notes per page) for Articulation, Language, and Fluency.
● Evaluation Tracker
: this allows you to write in where you are in evaluations to keep due dates from sneaking up on you!
● Screening Tracker
: record if the student passed, failed, or needs to be re-screened, along with the date, screening site, and areas of concern.
● CEU tracker
: keep track of the many continuing education hours we need! This is an Excel sheet that automatically tracks each hour for ASHA, State licensure, and "Other" categories (EI, Teaching Certification, etc), as well as allowing you to note if the hours will count for the CE Registry. * This is also available as a stand-alone product here
● Attendance tracker
: Excel file that includes rows for 60 students. You can include all students on one page or have one for each site you visit. I simply note each time I see a student with the number (1-4, etc) so I can easily check if a student has been seen for the required time. Weekend days are grayed out for ease of use. 2 pages (30 rows each) for each month from August 2017-July 2018, updated every year.
● Progress Report Tracker
: Room for 4 dates at the top, with 2 columns under each date so you can note when each report is done and when it is printed so none slip through the cracks!
