My Editable Student Data Forms allow you to easily document on one page how students perform on assessments throughout the school year.
This form serves as the “cover sheet” to each students’ section of my Student Data Binder. Whenever I meet with a student’s parents to discuss academic progress, I flip to that student’s section in my Student Data Binder and refer to this form.
This form will give you an overall picture of students' performance on an individual basis.
If using one of the pre-made forms, simply complete the top portion with your information. Also complete the “On Level” fields with the level or “cut-off score” expected for each quarter or benchmark. This feature helps put students’ scores in perspective for parents because it will inform them of the level or proficiency expected for their child to be considered “on grade level” for different times of the school year.
Refer to this Student Data Form during RTI meetings and Parent Teacher Conferences. At the end of the school year, you can even pass it on to the teacher your students will have the following year to give him/her a snapshot of how students performed in your class on assessments.
Note: The term LEP refers to Limited English Proficient students
(ESL or ELL students).
