This is a student assessment form that I developed when working 1-on-1 with ESL students. To use it, I asked questions of students during our first meeting and mark the grammatical and pronunciation problems I observed. I also wrote in the student's goals and timeline.

This is an excellent form to use for private tutoring and ESL classes. It can also be shared with students so they have an idea of what areas you need to work on together.
