This blank accommodations chart has been most helpful to me when sorting through student accommodations in the form of Individual Education Plans, 504s and Individual Learning Plans, specifically at the beginning of the school year and any time I have a new population of students (semester or quarter break). I have been using this chart for the past 3 school years and cannot remember how I ever worked through the school years without it.
At the beginning of the year, I take all of the materials regarding students with special services (IEPs, 504s, ILPs, etc.) and organize the information in this easy to see/use chart. I have used this chart both with my Special Education populations as well as my English Language Learning populations.
This chart is most helpful when writing modified tests, making seating charts, and any time that I need a quick glance at what individual students need without having to read through everything in the IEP, ILP or 504s.
