This sheet helps teachers keep students accountable for their behavior. Teachers create two specific behavior goals for an individual student. The student will track his/her daily progress towards those goals throughout the week. The sheet allows the student to reflect on each goal individually each day. Teachers can use this sheet to communicate with parents and also track progress over time. Because the goals are editable, teachers can fine-tune or tweak the goals as students begin showing progress.
This is an editable sheet. Both the title and the goals can be edited to allow it to be personalize for an individual student. To edit the desired text, click on it, and a text box will appear allowing you to type whatever text you’d like. You can modify the font and size in the Home tab settings if needed. Please make sure that you have Powerpoint so that you can access this resource!
There are two versions of this sheet. The first version has a blank area for the student to write a reflection of his/her progress toward each goal that day. The second version has happy-face, medium-face, and sad-face symbols under each goal for the student to circle. Both versions have a Teacher Comment section so teachers can communicate to parents or make notes to the student.
