I use this to organize my special education caseload. At the beginning of the year I type all the kids names and tentative information in the appropriate boxes. This really helps me to stay organized and make sure I am on time! My district has us follow certain steps that yours may not, so you can edit this document!
**Description of each subtitle:
Last: Student last name
First: Student first name
ID: Student identification number
Gr. Student grade level
Annual: Date/year for student's annual due date
Tri: Date/year for student's triannual due date
Proposed: Date that the meeting has been proposed
Time: The proposed time meeting is scheduled for
Changes: I mark if any changes have been made to proposed date/time
Type: I mark annual or Tri
Trans: If translator is needed, I mark the date I confirmed one
AP: When the assessment plan has been sent or received
Notice: When the meeting notice was sent
Returned: When parent returned notice signed
WCJIV: If it is a tri, I check when I have completed academic assessment
Passport: We complete passports for our students, I mark when completed
DO: When I have sent all paperwork to the district office
F: when meeting is finalized
