I created this incentive chart for a student who was becoming more and more "handsy" with other students. It provided a visual, incentives, and positive praise for keeping her hands to herself, which was very motivating. It is editable, so you can edit it for any target behavior, as well as for any time periods/academic subjects. You can also edit the visuals to fit your needs.
Directions: Print and laminate. I put the charts back to back for morning/afternoon use. Use a dry erase marker to draw a smiley face if the student exhibited the target behavior within that time frame. After "x" amount of smiley faces, they choose the reward. The student can choose the reward beforehand or after. You can cut the rewards out and put Velcro on if you want, and put hard Velcro on empty blank on "I need __ :) to get ___" area. You may also cut that sentence off and put it under the second chart if you'd like. It's editable so it's up to you!
My student simply chose from the reward menu when she earned her five smiley faces. Feel free to add any incentives that are motivating for your student, or change the ones I have.
