Editable Reading Intervention Template for Data and Flexible Grouping
This editable reading intervention template is an Excel document designed to help use reading test scores to easily see what students need intervention and in what areas (also can show what students need enrichment). I take this data sheet to every data team meeting to help us quickly see how students are performing and their progress. Read the included directions below for a more detailed description:
You can use this to record all students' reading test grades or just those that need intervention (based on percentage or repeated skill areas that they continue to struggle in). The V C R G S stand for concept strands consistently on the reading test (Vocabulary, Comprehension, Response, Grammar, and Spelling). This can be edited to fit the concepts typically found on your reading tests. I circle or highlight the letters for the areas that are weak and need extra attention. These are the areas that I use to form my flexible grouping for intervention time. I also use a combination of on-level and below level practice sheets from these categories to make a practice packet. This packet is assigned to all students as extra practice as extra credit and is required by students performing at risk. I meet with these students as a small group the day the packet is assigned and have them do 2 problems from each page just to get a feel for it and get help if needed before they take it home (I always mark pages that need to most practice based on the previous test and tell students to start on those first). I have the group sit together so I can easily monitor how they are all working. Often I can see areas that most students are struggling with and direct everyone's attention to that problem or sheet to go over it together. My teaching team found this sheet and method for creating our intervention to be very helpful in raising students' performance. The results were almost immediate for many. Parents received a note about the assigned practice packet, their child's current test grade, and the areas that need the most practice. The parent note is required to be signed and returned the next day (a place is available to check off when it's returned) and the packet is then returned completed by the day of the next test (a place is also available to check when completed). Many parents voiced that this really helped them know how to help their child.
We have also started to use this sheet to highlight students that typically perform very well on the entire test or in selected categories. When the practice packet is passed out to the class, these students get enrichment (beyond level) practice to do instead. Sometimes this means just replacing a few of the sheets from the practice packet and other times it means an entirely different set of practice from the class.
