These self-rating forms for SLPs and other special education teachers were modified on 2/18 and are now editable for text in adobe.
The Common Core Standards and Marzano's taxonomy place a valuable emphasis on having our students identify their learning goals and rate and monitor their own performance and progress. Yet, it can be difficult to effectively have them self-rate...we have to take our skill set beyond asking them "how did you do?" It is with this challenge in mind that these "self-rating forms for SLPs :were created.
These self-rating forms can be used in a variety of different ways: to have students assess their articulation productions (on target, close, incorrect?) their use of fluency enhancing strategies, and their responses to structured language probes. Many of the forms are open-ended enough that they can be used by non-SLP's to have students assess their performance in other academic, social or behavioral areas. These forms offer opportunities for students to identify their goal in therapy, assess their responses, determine their step on the speech or fluency ladder, and even assess the amount of effort they put into a task. So many chances to facilitate executive functioning skills and meta-cognition!
The forms are intended for preschool through early elementary.
