This editable behavior self reflection gives a space for students to score themselves on each criteria as well as the teacher to give them a score. This is such a great resource for conferences, since it really shows how the students view their behavior compared to how the teacher does.
I usually use the following scoring system:
1- Rarely/Never/Needs a lot of reminders
2- Sometimes/Needs some reminders
3 - Usually/Most of the time/ Rarely needs reminders
4- All the time/ Never needs reminders
