I just wanted to make a quick accountability sheet for my third graders to work in each day when they are in their read-to-self centers. This allows them to read freely while allowing me to hold them accountable. The example portion can be changed, I just always use Wayside School because it is the first book I read with my class each year. I have my students turn this into our "reading" work folder and pass it back out each day before centers. At the end of the week I like to have students do a quick hands-up pair-up to share all they have read this week!
