This is a weekly report that I used in my classroom. I gave all students a new one each week and sent it home with them on Friday. They were expected to bring it back by Tuesday morning with a parent signature. It was a lot of work at first but once students got the hang of it, it was easy and worked really well! This would work well as an individual behavior sheet as well if you didn't want to use it for your whole class. I used it in my 2nd grade class but it would work well with any grades really as long as you tweaked it a little ( I uploaded it as a .pptx so you can do so! )
-Homework check/reading log: I would walk around each morning during breakfast or the first few minutes of the day and check this section off
Behavior section: At the end of the day each student would color the square that matched where they were for the day. I would have team captains check and make sure that all students did this correctly and honestly.
Teacher comments: I had specials at the end of the day on Friday so this was a perfect time for me to fill in comments for every student, or you could just do it throughout the week.
Student self-assessment: students assessed their own behavior for the week. This part was really important and we had a lot of discussions about what each rating looked like.
I spent a lot of time at the beginning of the year going over these reports and making sure that students understood each section. I explained the reports to parents during back-to-school night and classroom newsletters. I encouraged parents to discuss the weekly reports with their child at home.
