We hold a transition meeting in January for our students who will be leaving us for middle school in the fall. Typically, the parents, the classroom teacher, IEP team, and, if possible, the new middle school teacher are invited to attend.
Prior to our meeting, we arrange a tour of the middle school for the parents. This gives them the opportunity to see what the school looks like and ask any questions they may have. Included in this packet is a parent letter to invite them to visit their child’s new school. We have found that this eases any fears that they may have prior to the transition meeting.
After the tour we ask that the parents, current classroom teachers, and IEP team (speech pathologist, OT, PT, social worker, etc.) fill out the middle school transition notes included in this packet. We will have an opportunity to discuss what we feel are student strengths, weaknesses, etc. with the new teacher. After the meeting we make a final copy to be shared with the new IEP team.
In the spring, the students will have the opportunity to tour their new school. During their tour they will participate in a scavenger hunt to familiarize them with the school.
