This Employability Skills Progress Report is a great way to keep track of data on students' work performance on the job or in the classroom! It also provides a method for communicating students' weekly progress with parents!

I use this data sheet weekly in my Job Training Program (a job skills program for students with disabilities ages 16-22). In my classroom, we focus on the 5 P's (prompt, polite, positive, prepared, and productive) in order to teach students how to be a "good worker" in the community. Students participate in daily job rotations, and are graded at the end of the day using the 5 P's.
I use the data sheet to keep track of student progress in terms of the 5 P's. At the end of each day, students meet with me individually, or one of my paraprofessionals, to go over this sheet. Together, we decide whether or not students should earn a check mark next to each question. For each check mark students earn, they earn 1 point. If students earn all of their check marks, they would earn 20 points for the day.
On Fridays, after going over the data sheet, we complete the student survey together. This gives students a chance to talk about their week, what jobs they liked, didn't like, and why. Then I make a copy of the data sheet and survey to keep in the students' files, and send the original home to their parents.
This form has provided students with a way of self-monitoring their own work behavior. It also provides me with A LOT of relevant data for IEP writing and progress reporting, as well as age-appropriate transition data!
