I started at a school that had pretty much no transition information for me whatsoever this past year. This sheet is pretty much everything I'd want to know as a speech therapist about their communication (although I think it could apply to a special ed teacher, too). I'm in a high need school, so this is very AAC and behavior focused, but it's an editable document, so you can change it to meet your own needs.
The idea is that you can just put together a binder with all this information about your kids (even if you just circled the information about each, that would be great) and pass it on to the next teacher or therapist. (Obviously in a way that protects confidentiality.) Hope it helps ya'll!
