I use this book at the end of the piano school year. Students can complete the pages, and bring it home for their parents.
Page 1: Title page.
Page 2: "My Year in Piano" page. Students (or teacher) fill in the blanks about the student's favourite moments over the past school year. At the bottom, there is a blank to fill out what level/grade the student started the year in, and what level/grade they are at now.
Page 3: A page to state key learnings over the past year, any performances, and what levels were completed.
Page 4: Students can draw themselves at the piano. Or a picture can be put in from the past year (I paste in a picture from the recital, and then have the student write a few sentences).
Page 5: I have students play me one of their favourite pieces learned over the past year, and I make comments using this music performance assessment sheet.
Page 6: A page to write any comments to the students. I write summer well-wishes, and practice suggestions over the summer.
