The print version of End of the Year Surveys is now in digital format!
Encourage upper elementary and intermediate students to thoughtfully reflect on the school year with this surveys and reflections activity. Created with Google™ Forms, this easy-to-use & ready-to-use resource combines Likert scale-type and open-ended questions that ask students for their opinions on the physical classroom, class environment, content areas, likes and dislikes, personal growth, and much more! Surveys are short (6-12 questions) and consist of thoughtful questions that are specific and tailored to each subject and topic. All responses are automatically transferred to a spreadsheet in Google™ Sheets, which you can use to organize, sort, and analyze data.
This resource includes the following surveys:

For your convenience, the resource includes each survey as a separate Google™ Form as well as one comprehensive survey with all 9 surveys organized into individual sections. In addition, a resource introduction with sharing instructions and suggested use is included.
