Self-assessment document designed to give teachers information on student readiness in various language arts domains, and to give students opportunities for goal setting.
Includes a 6-point scale for self-scoring in various skills related to four domains: Reading Fiction, Reading Nonfiction, Writing, and Grammar. Also includes a goal-setting form for students to set personal goals for each domain. Documents are easily customizable.
As a teacher, I typically collect the surveys, but have students keep the goal-setting sheet in order to refer to it later.
