Product Description
Save time by downloading my essential end-of-the-year (EOY) forms. Each form is easy to use and can be easily edited to fit your needs. Below is a bit more information on how each essential EOY form can be used.

As the school year winds down, teachers need to prepare class lists for outgoing students. Therefore, the class list spreadsheet (Excel) is the perfect tool to organize students and ensure next grade level teachers have balanced class rosters.

Also, it is important teachers help the next grade teachers better plan for the students they have not yet met. Student information cards (Word) can be filled out and passed on to teachers. Student information cards provide a way to summarize critical information needed to begin teaching students. For example, there are areas to record district/state test scores, special education information, ENL level, and more! The teacher comment section can be used to provide a couple of effective strategies in terms of academics and/or behaviors for the student listed on the card.

Not only are student end-of-the-year forms such as those mentioned above essential, but forms that help principals make informed decisions are essential to ending the school year on the right note. Principals can provide all staff with the position preference sheet (PPT). This sheet provides principals with a better picture of who would like to move positions, grade levels, are planning on retiring, etc. The backside of the sheet is a space for staff to note what committees they serve on, clubs they run, and/or sports they coach. If a staff member desires to drop one or more of these responsibilities or wishes to add an extra duty, this template provides the opportunity for them to make that known.

Lastly, I have included a generic staff accountability checklist for any task assigned to certified and/or classified staff as well as a checklist specific for end-of-the-year classroom teacher checklist for organizing materials, returning textbooks, and packing up classroom equipment. Again, like all the other forms, these forms can also be modified to fit your specific building needs.
