Individual Profile of Progress for chapter 3 & 4. This form matches the exit slips that we also created to help us identify student progress on specific targets. Students can use these to set goals at the start of an unit. Parents see these at a start to see what our teaching focus is for this unit. At the end of the unit, we fill these out for each student based on the evidence they have shown on the exit slips and final assessment. These then go home as a means of communication with families.
