The Executive Functions Checklist is an essential document for IEP meetings, parent-teacher conferences, and student service team meetings. This form is a handy resource to highlight areas of strength and weakness for students who have problems with executive disorders.
This checklist is divided into these categories:
- Flexible Thinking
- Working Memory
- Planning and Prioritizing
- Task Initiation and Completion
Behavior Regulation Skills
- Impulse Control
- Emotional Control
- Sustained Attention
Prior to meetings, complete this checklist using a +/- to indicate whether the student can access these skills when needed. I like to copy this form in different colors for each quarter. Use this form to show progress and present a more complete picture of students. A Comments section at the end allows for additional information to be included.
Be more prepared for meetings. A must-have for teachers.
