This informal questionnaire is designed to give teachers and educators a deeper look at their student's executive functioning skills. A detailed definition of executive functions is provided as well as resources and sources for further research in the area if desired. Questions are designed for caregivers or other educators to complete to provide the teacher with detailed and categorical info about the student's use of inner dialogue that is needed to complete tasks and manage time and assignments. This questionaire is useful when preparing for teacher conferences and can even be completed by teacher and or student if age is appropriate for self reflection. Categories included are as follows: Social Awareness and Perspective Taking, Organization, Time Management and Planning, Attention, Impulse Control, Task Initiation, Task Persistence, Self-Monitoring, and Shifting/Flexibility. Additional questions are included for person completing to provide a narrative description of skills.
