These multiplication exit slips (ticket to leave) are used as a quick assessment to determine whether or not students complete the task.

Directions: Make enough copies of the exit slip page (there are 20 on each page). Cut the exit slips and put them in a baggy with the same exit slips. There are 5 different assessments, so put each exit slip in a bag with the same exit slip. When you decide to give an exit slip have the students complete the slip and bring it to you. I put them in two piles (one for students who have mastered the concept and one for those who need help). Then I record the results on a grade sheet.

I like to use these quick ongoing assessments to determine what concepts to teach during small group math instruction, what type of homework needs to be used for additional practice and how much time to devote to a specific skill. I like this because then I am not waiting until an end of unit assessment to discover that half the class didn't understand a specific concept. Also, I tend to get behind on providing feedback on tests and this is a great way for me to give students immediate feedback so they have the time they need to correct any misconceptions.

I make several copies of each exit slip so I can use them throughout the year. I have a baggie of exit slips for many different concepts. I have a checklist and I mark if the students still need help with the concept so I can support them as needed. It also helps because I can move students forward once they master the concept.
