This exit slip can be printed out (multiple pages per sheet) and then photocopied to use to allow students to reflect and self-assess after a cooperative learning group work assignment. The purpose of this is to help students improve their cooperative learning skills and to make the practice of self-assessment a habit. It can also be used by the teacher as an assessment tool to evaluate group work and participation. These exit slips can be used to document the intangible aspect of cooperative learning.
It was designed with secondary students in mind, but certainly could be modified for elementary school.
