In my classroom, I use these exit slips at the end of the period. I print these slips out on green, yellow, and red paper. The students choose which color they feel represents their level of understanding for the day. If they understood the material, they would choose the green card. If they understood some, they would choose the yellow. If they need help, they would choose the red card. The students finish the sentences on the exit slips and turn them in at the end of class.
