I created these exit slips when doing my first practicum when I was doing the numeracy unit for grade one.
It does consist of six pages:
Page 1: Students are posed the question, "How many fingers are on four hands?" Students will have to show their work by numbering each hand.
Page 2: Is a subtraction word problem ("5 little piggies standing around a gate, 3 got out, and then there were _____.")
Page 3: Students are to make the corrections in tallies, addition/subtraction equations and based ten blocks.
Page 4: Students will have to identify numbers in various pictorially and concrete terms.
Page 5: Students are to count by five by labeling a clock.
Page 6: Students are given four numbers and to draw the number in base ten blocks.
There is two on a page. I would copy the exit slips a various colored paper which students enjoyed!
