Students should circulate around the room and question classmates in Spanish about their extracurricular activities. The goal is to complete the worksheet by identifying one classmate to fill in each square (the bottom-right square can be filled in with an activity mentioned that's not listed elsewhere). Once finished, students are asked to share 1-2 pieces of information about classmates. If desired, you could make this into a game where the student who completes the form most quickly is the winner.
